Acknowledgment of Risks and Acceptance of Responsibility
I, the undersigned, acknowledge that participation in activities at Plock Tennis Club, including the use of tennis courts and ball training machines, involves inherent physical, environmental, and health risks. These risks include, but are not limited to:
Physical injuries such as sprains, fractures, muscle strains, concussions, or more severe injuries like internal damage or death.
Environmental risks including severe weather, slippery surfaces, and wildlife exposure.
Health risks from pre-existing conditions or overexertion, and the possibility of delays in accessing emergency medical care.
Psychological risks arising from stress, anxiety, or trauma during activities.
Social risks, including exposure to contagious illnesses or conflicts with other participants.
I voluntarily assume all risks associated with my participation in activities at Plock Tennis Club. I understand that I may withdraw from activities at any time but that my participation is entirely at my own risk. I accept full responsibility for any injuries, damages, or losses that may result from my involvement.
Release of Liability and Indemnification
In consideration for being allowed to participate, I, and my legal representatives, hereby release, waive, and discharge Plock Tennis Club, its business owner, officers, staff, volunteers, and affiliates (the Released Parties) from any and all claims, liabilities, or causes of action, whether arising from negligence or other causes, that result from my participation in activities.
I agree to indemnify and hold harmless the Released Parties from any claims, damages, or legal costs arising from my actions during the activity, including claims brought by third parties.
Medical Consent and Emergency Treatment
In the event of an emergency, I authorize Plock Tennis Club to obtain medical treatment on my behalf, understanding that I will be responsible for all associated costs, including emergency transport. I confirm that I am physically fit to participate and have disclosed any relevant medical information.
Media Consent
I give permission for Plock Tennis Club to use photographs, video recordings, or other media of my participation for promotional, educational, or marketing purposes without compensation.
Governing Law and Severability
This waiver is governed by the laws of Province or Country. Any disputes will be resolved in the courts of Province or Country. If any provision of this waiver is found to be unenforceable, the remaining provisions will remain in full force.
Participant Acknowledgment
I acknowledge that I have read and understood the terms of this Liability Waiver and Consent Form. I voluntarily agree to its terms and understand that I am waiving certain legal rights, including the right to pursue legal action for injuries or damages related to my participation.
